Can a ceiling fan go where a light fixture is now?

Yes, and this is the most common fan job there is — but the box behind the fixture almost always needs replacing with a fan-rated one. That is part of the standard install, not an extra discovered later.

How big a bath fan do I need?

Roughly one CFM per square foot for a standard bath, more for high ceilings or a separate shower room. Oversizing slightly and running it on a timer beats an undersized unit running forever.

My bath fan runs but the mirror still fogs. Why?

Either it is ducted nowhere (attic dumping is epidemic in houses here), the damper is stuck, or the duct run is crushed. A look in the attic answers it in five minutes.
